Understanding homeowners policy forms

Homeowners insurance in the United States is sold under standardized policy forms developed by the Insurance Services Office (ISO). Each form has a specific code (HO-1 through HO-8) that determines what perils are covered, on what basis (named perils vs open perils), and at what coverage levels.

About 80% of US homeowners have an HO-3 (Special Form) policy. HO-5 is a premium upgrade with broader coverage. HO-6 is for condos. HO-4 is for renters. HO-8 is a modified form for older homes that don't meet HO-3 underwriting standards.

Important caveat: Even within the same form, specific policies vary by carrier, state, and the riders added. Use this tool for general orientation only — your specific coverage details, exclusions, and limits should be confirmed with your insurance agent or by reviewing your declarations page.
